Short-Term Disability Benefits
- Coverage provided by the Bank
- Up to twelve-week benefit duration after a seven calendar day elimination

Long-Term Disability Benefits
- Bank-paid Base benefit equal to 40% of monthly earnings
- Voluntary Associate Buy-up of additional 26.7%, for a total benefit equal to 66.7%
- Guaranteed approval for both base and buy-up
- Benefits begin after 90 consecutive days of disability (coordinates with STD)
Long Term Care Insurance

401(k) Plan
- Daily valuation plan
- Associates may contribute up to 100 percent of pay not to exceed IRS
